I recommend, if you haven't read it yet, Jonathan Schell's Fate of the Earth, which contains the science behind why even a limited nuclear exchange, as few as 15 or so blasts, will render the earth uninhabitable for several years from the ensuing nuclear winter.

So I guess people can enter shelters if they wish, but when they dig themselves out they'll emerge into a freezing landscape with nothing to eat.
